School Supply List

Medication Authorization Form

This form is to be used for those students that will need to take medication during the school day.  It is to be completed and signed by the student’s physician and the parents.  It will be kept on file in the school office.

Parking Pass Agreement Form

Parking permit forms will be sent digitally from Mrs. Kupferer on August 1.  If your child receives his or her license or begins driving to school sometime after the start of the school year, please contact Mrs. Kuepfer at [email protected] for information on purchasing a parking pass.

Pre-Arranged Absence Form

This form is to be used by those students that will be missing school for family trips, vacations, extended vacations, non-school sponsored athletics competitions, etc.  It does not need to be completed for doctor visits. The form must be completed, signed by each teacher, and turned in to the office no later than 5 school days before the student’s absence.

College Visit Request Form

College visits are encouraged. Pre-arranged absences may be granted for college visits for juniors and seniors. Notice should be made in writing two school days in advance of the absence and the College Day Request Form must be completed BEFORE the absence.
